Our Filler Philosophy
Conservative, Comprehensive, Accountable
At PM Aesthetics, we approach filler with a medicine-first philosophy that prioritizes natural results, structural integrity, and patient safety over sales volume.
What Makes Our Approach Different
1. We'll Refuse to Overfill You
Unlike commission-based practices where providers earn bonuses for syringe volume, our salaried medical providers can (and will) say "no" when you request more filler than is clinically appropriate.
We've turned down patients requesting:

Excessive lip volume that would look unnatural

Filler in areas where muscle stimulation or structural treatments would work better

Treatment frequency that indicates body dysmorphia or filler dependency
This is medicine, not customer service. Your long-term outcome matters more than today's sale.
2. We Document Everything
Before every filler treatment, you'll complete a comprehensive treatment plan that documents:

Your stated goals in your own words

Our provider's clinical recommendation (including areas we recommend AGAINST treating)

Your final decision (whether you followed our recommendation, requested more conservative, or requested more aggressive)

Your acknowledgment of settling timelines and responsibility for requested outcomes
Why this matters: It protects both of us. You know exactly what you're consenting to. We have documentation of what you requested. If issues arise, there's no ambiguity about who decided what.
3. Mandatory 2-Week Assessment
Filler takes 14 days to fully settle due to swelling, enzymatic breakdown, and tissue integration. Judging results before 2 weeks leads to unnecessary panic and premature dissolution.
Our policy:

Every filler patient receives a mandatory 2-week follow-up appointment

We evaluate results against pre-treatment photos and your signed treatment plan

If WE made an error → we fix it at no charge (dissolution or additional filler, your choice)

If results match your requested treatment plan → you're responsible for any changes
If you don't attend your 2-week assessment, you forfeit free correction eligibility. We can't fix issues we don't know about within the appropriate clinical timeline.
4. Clear Dissolution Responsibility
We provide FREE dissolution if: ✓ Provider technique error caused objectively visible issue (confirmed in photos and by multiple providers) ✓ Medical complication occurred (vascular compromise, infection, true nodule) ✓ You followed our recommended treatment plan exactly ✓ Issue identified at 2-week assessment and confirmed by provider as clinically significant
You pay standard dissolution fees if: ✗ You requested more filler than we recommended (documented in your treatment plan) ✗ You requested filler in areas we advised against (documented in your treatment plan) ✗ You're dissatisfied with results you specifically requested and consented to ✗ You skipped your 2-week assessment and present >30 days later ✗ You want dissolution before 2-week settling period (except medical emergency) ✗ You perceive asymmetry or imperfection that is not objectively visible in photos or to providers (imperceptible to others)
Why this matters: We stand behind our clinical work when objective errors occur. However, some patients perceive asymmetries or imperfections that are not visible to providers, not visible in photographs, and fall within normal human facial variation.
Faces are naturally asymmetric—perfectly symmetric faces don't exist. When patients focus on millimeter-level differences that no one else can see, this represents subjective concern rather than medical error. We will not provide free product for medically unnecessary corrections of imperceptible variations.
We approach these situations with empathy and without judgment. If you perceive an issue after reviewing side-by-side photos with your provider, we'll correct it, but standard fees apply for subjective, non-objective concerns.
Dissolution pricing: $600 per vial (free if provider-caused per above criteria)
Why We Use Multiple Filler Brands
We carry FDA-approved fillers from Allergan (Juvéderm family) and Merz (Belotero, Radiesse). Different fillers have different rheological properties suited to different anatomical locations:

High G' (cohesive) fillers (Voluma, Radiesse): Deep structural support, cheeks, jawline

Medium G' fillers (Vollure, Ultra Plus): Moderate depth, nasolabial folds, marionette lines

Low G' (soft) fillers (Volbella, Belotero): Superficial placement, lips, fine lines

Biostimulatory fillers (diluted Radiesse): Pan-facial collagen stimulation
We choose filler based on anatomy and goal, not profit margin. All our fillers are FDA-approved, sourced from authorized distributors, and stored per manufacturer guidelines.
If you've had "lumps" with Juvéderm at another practice, the issue was likely:

Wrong filler type for the location (high G' filler placed superficially)

Normal swelling judged too early (<2 weeks)
Filler brand rarely causes lumps. Technique and placement do.
