Your provider will assess:

Can I Choose My Own Filler?
You're welcome to express preferences, but your provider has final say based on what's clinically appropriate for your anatomy.

Why? Using the wrong filler can create:

  • Lumps or irregularities (too thick for delicate tissue)

  • Duck lips or "shelf" appearance (too much projection)

  • Unnatural texture (product doesn't integrate well)

  • Excessive swelling (wrong formula for your tissue type)

We want you to love your results, which means using the right product for the right purpose.

If you have concerns about our recommendation, we'll explain our reasoning and show you examples of results from each product. But we won't compromise your outcome to accommodate a preference that's not clinically sound.

What About Other Brands?
We only use Allergan (Juvéderm) and Merz (Belotero) products. We do not use:

  • Restylane (different viscosity, not our preferred product line for lips)

  • RHA fillers (excellent for facial movement areas, but we prefer Juvéderm/Belotero for lips)

  • Compounded or off-brand fillers (safety and quality concerns)

  • Permanent or semi-permanent fillers (too risky, cannot be reversed)

When We Say No to Lip Filler:
Your Safety Over Sales

At PM Aesthetics, our providers have both the authority and ethical obligation to decline lip filler treatments that aren't in your best medical or aesthetic interest. This is a medical practice, not a cosmetics counter, and sometimes the most responsible care we can provide is refusing treatment.

​

We Will Not Perform Lip Filler If:
You're requesting excessive volume or unnatural proportions. If you're asking for your third syringe this year and your lips no longer match your facial anatomy, we'll have an honest conversation instead of handing you a consent form. Overfilled lips that compromise facial harmony aren't just an aesthetic concern, they're a medical one.

​

You want treatment too frequently. Requesting top-ups every 2-3 months when filler should last 12-18 months signals either abnormal metabolism (which needs investigation, not more filler) or unrealistic expectations. We won't enable overconsumption or dependency behaviors.

​

You have an active cold sore or infection. Injecting into infected tissue can spread infection and cause serious complications. If you have any signs of HSV-1 outbreak, your appointment will be rescheduled, no exceptions.

​

You're trying to correct botched work from unqualified providers. If you received injections from unlicensed practitioners, used black-market products, or have unknown substances in your lips, we may not be able to safely correct the damage. We'll assess honestly but won't make promises we can't keep.

​

You show signs of body dysmorphia. If you're fixated on imperceptible "flaws" or requesting changes that would create disproportion rather than enhancement, we may recommend speaking with a mental health professional instead of providing filler.

​

You've been refused elsewhere. If other reputable providers have declined treatment, there's likely a valid clinical reason. We're not the "yes" you're looking for after collecting refusals from qualified practitioners.

​

You're under 21 without clear medical indication. Young lips rarely need volume enhancement. We won't inject lips just because it's trending on social media.

​

What Happens When We Decline:

  • Clear explanation based on clinical reasoning and safety data

  • Alternative treatments if appropriate

  • Referral to specialists if we genuinely cannot help

  • No judgment, but also no compromise on standards

 

Why This Matters:
Most med spas will sell you whatever you ask for, whenever you want it. Their business model depends on it. Ours doesn't. We're not commission-based, franchise-owned, or investor-driven. We're medical professionals whose licenses, reputations, and ethics matter more than monthly sales targets.

Your right: You're free to seek treatment elsewhere if we decline.
Our responsibility: We'd rather you leave disappointed than leave with a complication, unnatural result, or lasting regret.

Lip Filler & Cold Sores (HSV-1):
What You Need to Know

If you have a history of cold sores (oral herpes/HSV-1), lip filler can trigger an outbreak. This is completely manageable with proper prevention, but you must tell us before your appointment.

​

Why Lip Filler Can Trigger Cold Sores
Cold sores are caused by the herpes simplex virus (HSV-1), which lies dormant in nerve cells near your lips. Trauma to the lips, including needle injections, can reactivate the virus and cause an outbreak.

​

This doesn't mean you can't get lip filler. It just means we need to take precautions.

​

Who Needs Antiviral Medication?
You need antiviral prophylaxis (preventive medication) if:

  • You've ever had a cold sore, even once, even years ago

  • You get cold sores occasionally or frequently

  • You're prone to outbreaks during stress, illness, or sun exposure

  • You've had oral herpes reactivated by dental work or other lip trauma

 

"I haven't had a cold sore in years" does NOT mean you're safe to skip medication. The virus is still dormant and can reactivate with lip filler.

​

What Happens If You Don't Take Preventive Medication?
If you have a history of HSV-1 and don't take antiviral medication before lip filler, you risk:

  • Painful, severe cold sore outbreak within 2-7 days of treatment

  • Blistering and scabbing over freshly injected lips

  • Prolonged healing time (2-3 weeks instead of 7-10 days)

  • Potential scarring or uneven filler results

  • Delayed ability to assess final results (swelling + outbreak makes it hard to evaluate)

 

Prevention is always easier than treatment.

How to Prevent Cold Sore Outbreaks with Lip Filler
Step 1: Tell Us About Your History

When booking your appointment or during your consultation, disclose any history of cold sores, even if it was years ago or "just once."

​

We need to know so we can:

  • Prescribe antiviral medication in advance

  • Schedule your appointment appropriately (you'll need to start medication before treatment)

  • Monitor you for signs of outbreak after treatment

 

Do not withhold this information out of embarrassment or because you think it's not relevant. It's medically necessary for your safety and optimal results.

​

Step 2: Take Antiviral Medication

We'll prescribe Valacyclovir (Valtrex) or a similar antiviral to suppress the virus before and after your lip filler appointment.

​

Typical protocol:

  • Start taking medication 1-2 days before your lip filler appointment

  • Continue taking medication for 5-7 days after treatment

  • Exact dosing and duration will be determined by your provider

 

You must pick up your prescription and start taking it BEFORE your appointment. We cannot inject your lips if you haven't started the medication.

Step 3: Avoid Other Triggers

Even with antiviral medication, you should minimize other HSV-1 triggers around your appointment:

  • Avoid excessive sun exposure (UV light can reactivate HSV-1)

  • Manage stress and get adequate sleep

  • Stay hydrated and maintain a healthy immune system

  • Avoid other lip trauma (aggressive exfoliation, dental work, etc.)

 

What If I'm Already Having a Cold Sore Outbreak?
We will not perform lip filler if you have an active cold sore outbreak.

​

If you have blisters, sores, tingling, or any signs of an active HSV-1 infection, your appointment will need to be rescheduled until:

  • The outbreak has completely healed

  • You've been on antiviral medication

  • Your provider clears you for treatment

 

Do not try to hide an active outbreak. Injecting filler into infected tissue can:

  • Spread the infection

  • Cause severe complications

  • Compromise your results

  • Put our providers and other patients at risk

 

Call us immediately if you develop symptoms before your scheduled appointment.

 

What If I've Never Had a Cold Sore?
You likely don't need antiviral medication, but there are exceptions:

  • If you've been exposed to HSV-1 but never had symptoms (asymptomatic carrier)

  • If you're immunocompromised or have a history of frequent infections

  • If your provider determines you're at higher risk for any reason

 

Your provider will assess your individual risk during your consultation.

Will Insurance Cover Antiviral Medication?
Maybe. Antiviral prophylaxis for cosmetic procedures is sometimes covered by insurance, but not always.

 

  • With insurance: Valacyclovir typically costs $10-30 for a short course

  • Without insurance: Generic valacyclovir costs $20-50 at most pharmacies

  • GoodRx or discount programs can reduce cost further

 

This is a small investment to protect your $699+ lip filler investment.

What to Expect: The Lip Filler Experience

Before Your Appointment

  • Avoid blood thinners (ibuprofen, aspirin, alcohol) 24-48 hours before treatment (unless medically necessary)

  • Avoid excessive sun exposure or tanning beds

  • Arrive with clean skin (no makeup on lips)

  • Bring photos of lip shapes you like (Pinterest, Instagram, etc.)

After Your Appointment

Immediate Effects:

  • Lips will appear swollen (this is normal and peaks at 24-48 hours)

Our Split Syringe Policy:
Why We Don't Offer Half-Syringe Pricing

We're often asked if we can split a syringe between two patients or save half a syringe for later use at a reduced price. The answer is no, and here's why.

What "Split Syringes" Actually Means
When a practice offers to split a syringe or save half for later, they are:

  • Opening a sterile syringe and storing the unused portion in a refrigerator

  • Reusing that opened syringe on a second patient (or the same patient days/weeks later)

  • Violating manufacturer safety protocols and FDA sterility guidelines

 

Once a syringe is opened, it is no longer sterile, even if stored in a fridge. Bacteria and contaminants can enter the moment it's exposed to air or comes into contact with skin or needles.

The Risks of Reusing Opened Syringes
Reusing an opened filler syringe can lead to:

  • Infection (bacterial contamination, abscesses, cellulitis)

  • Granulomas (inflammatory lumps requiring medical treatment)

What Allergan & Merz Say
Allergan and Merz, the pharmaceutical companies that manufacture the dermal fillers we use, explicitly prohibit storing and reusing opened syringes.

 

From their safety guidelines:

  • Syringes are single-use only

  • Once opened, any unused product must be discarded immediately

  • Storing opened syringes for later use voids all product warranties and safety guarantees

 

We follow these guidelines without exception.

 

Why Some Practices Offer Split Syringes Anyway
We understand, filler is expensive, and splitting syringes can make it seem more affordable. So why do some practices do it?

 

Because they prioritize profit over safety.

Offering split syringes allows practices to:

  • Most patients achieve beautiful, natural results with 1 syringe (1 mL) of filler. This is enough to add noticeable volume, enhance shape, and create balanced proportions without looking overdone.

    • The product used (Juvéderm lasts longer than Restylane in lips which is why we use  Juvéderm)

    Lip injections can trigger cold sore outbreaks in people with a history of HSV-1 (oral herpes). We'll prescribe Valtrex (valacyclovir) to take before and after treatment to prevent an outbreak.

    Let your provider know if you've ever had a cold sore—even years ago.

  • Wait 24 hours before applying lipstick, lip gloss, or any lip products. This allows the injection sites to heal and reduces infection risk.

    ​

    After 24 hours, you can resume normal makeup application.

  • Bruising and swelling are normal and expected.

    To minimize:

    • Avoid blood thinners (ibuprofen, aspirin, alcohol, fish oil) 48 hours before and after treatment

    • Use Arnica (topical cream or oral tablets) starting 2 days before treatment

    • Apply ice or cold compress for the first 24 hours

    • Sleep elevated (propped on pillows) the first night

     

    Swelling peaks at 24-48 hours and resolves within 7-14 days.
    Bruising typically fades within 7-10 days.

    If you have an event (wedding, photoshoot, etc.), book lip filler at least 2 weeks in advance.
