Lip Enhancement Options
We offer three approaches to enhancing your lips, each designed for different goals and budgets:
1. Lip Filler
What It Is:

Best For:
-
Adding volume to thin or aging lips
-
Enhancing lip shape and definition
-
Correcting asymmetry
-
Smoothing vertical lip lines (smoker's lines)
-
Creating a more youthful, hydrated appearance
How Long It Lasts:
12-18 months (depending on product and metabolism)
Pricing:
$699 per syringe (member pricing available)
Important: Do You Get Cold Sores?
If you've ever had a cold sore (even years ago), you need antiviral medication before lip filler to prevent an outbreak. Tell us when you book so we can send a prescription. Learn more.
2. Lip Flip
What It Is:
A small amount of neurotoxin (Botox or Xeomin) injected into the muscle above the upper lip, causing it to gently "flip" outward for a fuller appearance without adding volume.
Best For:
-
Subtle upper lip enhancement without filler
-
Softening a "gummy smile"
-
Creating the appearance of more visible upper lip
-
Patients new to lip enhancement who want to start conservatively
How Long It Lasts:
2-3 months
Pricing:
$69 (can be added to any lip filler treatment)

Choosing the Right Filler for Your Lips:
Our Product Selection Process
Not all lip fillers are the same. At PM Aesthetics, we don't use a one-size-fits-all approach. Your provider will select the best filler for your unique anatomy, goals, and desired outcome.
Here's how we choose:
The Fillers We Use for Lips
We exclusively use FDA-approved, pharmaceutical-grade hyaluronic acid fillers from Allergan (Juvéderm) and Merz Aesthetics (Belotero). Each product has different properties that make it ideal for specific goals.
1. Belotero Balance
Best For:
-
First-time lip filler patients who want subtle, natural enhancement
-
Thin lips that need soft volume without looking overdone
-
Patients who are nervous about "too much" filler
-
Smoothing fine vertical lip lines (smoker's lines)
-
Creating a naturally hydrated appearance
Why We Choose It: Belotero is the softest, most fluid filler we use. It integrates seamlessly into tissue, creating an ultra-natural feel and appearance. It's perfect for patients who want "your lips, but better" rather than dramatic volume.
Texture: Soft and smooth
Longevity: 9-12 months
Feel: Most natural, least detectable
2. Juvéderm Volbella
Best For:
-
Subtle, natural lip enhancement with minimal swelling
-
Smoothing fine vertical lip lines (perioral lines/smoker's lines)
-
Adding slight volume while maintaining natural texture
-
Patients who want the longest-lasting subtle result
-
Combining with other treatments (lip flip, other fillers)
Why We Choose It: Volbella is specifically designed for subtle lip enhancement and fine line smoothing. It uses Vycross technology (creates a smoother, longer-lasting gel) and provides natural-looking results with less swelling than other fillers. Perfect for patients who want enhancement that lasts but looks effortlessly natural.
Texture: Very smooth, low-swelling formula
Longevity: 12-18 months (longest-lasting subtle filler)
Feel: Exceptionally natural, soft
3. Juvéderm Ultra
Best For:
-
Balanced lip enhancement (moderate volume + definition)
-
Patients who want noticeable but natural results
-
Defining lip borders and creating shape
-
Adding volume to moderately thin lips
-
Most versatile option for average lip enhancement goals
Why We Choose It: Ultra is our go-to filler for most patients. It strikes the perfect balance between soft, natural feel and visible enhancement. It holds its shape well while still looking and feeling natural.
Texture: Smooth, slightly more structured than Belotero/Volbella
Longevity: 12-15 months
Feel: Natural with good structure
4. Juvéderm Ultra Plus
Best For:
-
Patients who want fuller, more dramatic volume
-
Lips that have lost significant volume due to aging
-
Creating projection and "pout"
-
Patients who've had filler before and want more impact
-
Balancing proportions (e.g., small top lip, fuller bottom lip)
Why We Choose It: Ultra Plus is thicker and more robust than Ultra, providing maximum volume and projection. It's ideal for patients who want noticeable fullness and aren't afraid of a bolder result.
Texture: Thicker, more volumizing
Longevity: 15-18 months
Feel: Fuller, more substantial (still natural, just more present)
How We Decide Which Filler to Use
Your provider will assess:
-
Your current lip anatomy (thin, moderate, or full)
-
Your skin thickness (delicate tissue = softer filler)
-
Your aesthetic goals (subtle vs. dramatic enhancement)
-
Your filler history (first-time vs. experienced patient)
-
Your lifestyle and budget (longevity vs. cost considerations)
-
Your tendency to swell (some fillers cause less swelling than others)
We may also combine fillers in one treatment:
-
Example: Volbella in the lip body for soft, natural volume + Ultra in the border for definition
-
Example: Belotero to smooth fine lines + Ultra for volume
-
Example: Ultra Plus for volume + Volbella to refine texture
This customized approach ensures your results look naturally you, not cookie-cutter.
Can I Choose My Own Filler?
You're welcome to express preferences, but your provider has final say based on what's clinically appropriate for your anatomy.
Why? Using the wrong filler can create:
-
Lumps or irregularities (too thick for delicate tissue)
-
Duck lips or "shelf" appearance (too much projection)
-
Unnatural texture (product doesn't integrate well)
-
Excessive swelling (wrong formula for your tissue type)
-
Shorter-than-expected results (product breaks down faster in wrong placement)
We want you to love your results, which means using the right product for the right purpose.
If you have concerns about our recommendation, we'll explain our reasoning and show you examples of results from each product. But we won't compromise your outcome to accommodate a preference that's not clinically sound.
What About Other Brands?
We only use Allergan (Juvéderm) and Merz (Belotero) products. We do not use:
-
Restylane (different viscosity, not our preferred product line for lips)
-
RHA fillers (excellent for facial movement areas, but we prefer Juvéderm/Belotero for lips)
-
Compounded or off-brand fillers (safety and quality concerns)
-
Permanent or semi-permanent fillers (too risky, cannot be reversed)
Our product selection is based on years of experience, advanced training, and consistent, predictable outcomes.
Questions About Product Selection?
During your consultation, your provider will explain which filler they recommend and why. We'll show you examples, discuss expected outcomes, and answer any questions about product choice.
Our goal: The right product, in the right place, for the right result.
When We Say No to Lip Filler:
Your Safety Over Sales
At PM Aesthetics, our providers have both the authority and ethical obligation to decline lip filler treatments that aren't in your best medical or aesthetic interest. This is a medical practice, not a cosmetics counter, and sometimes the most responsible care we can provide is refusing treatment.
We Will Not Perform Lip Filler If:
You're requesting excessive volume or unnatural proportions. If you're asking for your third syringe this year and your lips no longer match your facial anatomy, we'll have an honest conversation instead of handing you a consent form. Overfilled lips that compromise facial harmony aren't just an aesthetic concern, they're a medical one.
You want treatment too frequently. Requesting top-ups every 2-3 months when filler should last 12-18 months signals either abnormal metabolism (which needs investigation, not more filler) or unrealistic expectations. We won't enable overconsumption or dependency behaviors.
You have an active cold sore or infection. Injecting into infected tissue can spread infection and cause serious complications. If you have any signs of HSV-1 outbreak, your appointment will be rescheduled, no exceptions.
You're trying to correct botched work from unqualified providers. If you received injections from unlicensed practitioners, used black-market products, or have unknown substances in your lips, we may not be able to safely correct the damage. We'll assess honestly but won't make promises we can't keep.
You show signs of body dysmorphia. If you're fixated on imperceptible "flaws" or requesting changes that would create disproportion rather than enhancement, we may recommend speaking with a mental health professional instead of providing filler.
You've been refused elsewhere. If other reputable providers have declined treatment, there's likely a valid clinical reason. We're not the "yes" you're looking for after collecting refusals from qualified practitioners.
You're under 21 without clear medical indication. Young lips rarely need volume enhancement. We won't inject lips just because it's trending on social media.
What Happens When We Decline:
-
Clear explanation based on clinical reasoning and safety data
-
Alternative treatments if appropriate
-
Referral to specialists if we genuinely cannot help
-
No judgment, but also no compromise on standards
Why This Matters:
Most med spas will sell you whatever you ask for, whenever you want it. Their business model depends on it. Ours doesn't. We're not commission-based, franchise-owned, or investor-driven. We're medical professionals whose licenses, reputations, and ethics matter more than monthly sales targets.
Your right: You're free to seek treatment elsewhere if we decline.
Our responsibility: We'd rather you leave disappointed than leave with a complication, unnatural result, or lasting regret.
That's the PM Aesthetics difference: Medicine first. Sales never.
Lip Filler & Cold Sores (HSV-1):
What You Need to Know
If you have a history of cold sores (oral herpes/HSV-1), lip filler can trigger an outbreak. This is completely manageable with proper prevention, but you must tell us before your appointment.
Why Lip Filler Can Trigger Cold Sores
Cold sores are caused by the herpes simplex virus (HSV-1), which lies dormant in nerve cells near your lips. Trauma to the lips, including needle injections, can reactivate the virus and cause an outbreak.
This doesn't mean you can't get lip filler. It just means we need to take precautions.
Who Needs Antiviral Medication?
You need antiviral prophylaxis (preventive medication) if:
-
You've ever had a cold sore, even once, even years ago
-
You get cold sores occasionally or frequently
-
You're prone to outbreaks during stress, illness, or sun exposure
-
You've had oral herpes reactivated by dental work or other lip trauma
"I haven't had a cold sore in years" does NOT mean you're safe to skip medication. The virus is still dormant and can reactivate with lip filler.
What Happens If You Don't Take Preventive Medication?
If you have a history of HSV-1 and don't take antiviral medication before lip filler, you risk:
-
Painful, severe cold sore outbreak within 2-7 days of treatment
-
Blistering and scabbing over freshly injected lips
-
Prolonged healing time (2-3 weeks instead of 7-10 days)
-
Potential scarring or uneven filler results
-
Delayed ability to assess final results (swelling + outbreak makes it hard to evaluate)
Prevention is always easier than treatment.
How to Prevent Cold Sore Outbreaks with Lip Filler
Step 1: Tell Us About Your History
When booking your appointment or during your consultation, disclose any history of cold sores, even if it was years ago or "just once."
We need to know so we can:
-
Prescribe antiviral medication in advance
-
Schedule your appointment appropriately (you'll need to start medication before treatment)
-
Monitor you for signs of outbreak after treatment
Do not withhold this information out of embarrassment or because you think it's not relevant. It's medically necessary for your safety and optimal results.
Step 2: Take Antiviral Medication
We'll prescribe Valacyclovir (Valtrex) or a similar antiviral to suppress the virus before and after your lip filler appointment.
Typical protocol:
-
Start taking medication 1-2 days before your lip filler appointment
-
Continue taking medication for 5-7 days after treatment
-
Exact dosing and duration will be determined by your provider
You must pick up your prescription and start taking it BEFORE your appointment. We cannot inject your lips if you haven't started the medication.
Step 3: Avoid Other Triggers
Even with antiviral medication, you should minimize other HSV-1 triggers around your appointment:
-
Avoid excessive sun exposure (UV light can reactivate HSV-1)
-
Manage stress and get adequate sleep
-
Stay hydrated and maintain a healthy immune system
-
Avoid other lip trauma (aggressive exfoliation, dental work, etc.)
What If I'm Already Having a Cold Sore Outbreak?
We will not perform lip filler if you have an active cold sore outbreak.
If you have blisters, sores, tingling, or any signs of an active HSV-1 infection, your appointment will need to be rescheduled until:
-
The outbreak has completely healed
-
You've been on antiviral medication
-
Your provider clears you for treatment
Do not try to hide an active outbreak. Injecting filler into infected tissue can:
-
Spread the infection
-
Cause severe complications
-
Compromise your results
-
Put our providers and other patients at risk
Call us immediately if you develop symptoms before your scheduled appointment.
What If I've Never Had a Cold Sore?
You likely don't need antiviral medication, but there are exceptions:
-
If you've been exposed to HSV-1 but never had symptoms (asymptomatic carrier)
-
If you're immunocompromised or have a history of frequent infections
-
If your provider determines you're at higher risk for any reason
Your provider will assess your individual risk during your consultation.
How to Disclose Your Cold Sore History
Option 1: Tell us when you book
Mention it to our front desk staff when scheduling your appointment. They'll make a note in your chart and ensure your provider is aware.
Option 2: Call ahead if you've already booked
If you've already scheduled your lip filler appointment and realize you have a history of cold sores, call us immediately at:
Bedford, NH: (603) 341-7100
Lynnfield, MA: (781) 224-3334
We'll send a prescription to your pharmacy so you can start medication before your appointment.
Option 3: Disclose during your consultation
If you're coming in for a consultation first, let your provider know during your assessment. We'll send a prescription before scheduling your treatment.
Will Insurance Cover Antiviral Medication?
Maybe. Antiviral prophylaxis for cosmetic procedures is sometimes covered by insurance, but not always.
-
With insurance: Valacyclovir typically costs $10-30 for a short course
-
Without insurance: Generic valacyclovir costs $20-50 at most pharmacies
-
GoodRx or discount programs can reduce cost further
This is a small investment to protect your $699+ lip filler investment.
Bottom Line: Just Tell Us
We're not here to judge you for having cold sores. Up to 67% of people under 50 have HSV-1, it's incredibly common.
What we care about is:
-
Protecting your health
-
Preventing complications
-
Ensuring you get the best possible results
Disclosure is not optional. It's a medical necessity.
If you've ever had a cold sore, tell us before booking or at your consultation. We'll take care of the rest.
During Your Appointment (30-45 minutes)
Step 1: Consultation & Assessment
Your provider will assess your lip anatomy, discuss your goals, and explain what's realistically achievable for your unique features.
Step 2: Numbing
Topical numbing cream is applied to minimize discomfort. Most fillers also contain lidocaine for additional comfort during injection.
Step 3: Injection
Your provider will strategically place filler to enhance volume, define borders, and create balanced proportions. You'll be able to see and approve results in real-time.
Step 4: Assessment & Touch-Ups
Your provider will massage and shape the filler, ensuring symmetry and natural appearance.

Before & After: The PM Aesthetics Difference
Our Split Syringe Policy:
Why We Don't Offer Half-Syringe Pricing
We're often asked if we can split a syringe between two patients or save half a syringe for later use at a reduced price. The answer is no, and here's why.
What "Split Syringes" Actually Means
When a practice offers to split a syringe or save half for later, they are:
-
Opening a sterile syringe and storing the unused portion in a refrigerator
-
Reusing that opened syringe on a second patient (or the same patient days/weeks later)
-
Violating manufacturer safety protocols and FDA sterility guidelines
Once a syringe is opened, it is no longer sterile, even if stored in a fridge. Bacteria and contaminants can enter the moment it's exposed to air or comes into contact with skin or needles.
The Risks of Reusing Opened Syringes
Reusing an opened filler syringe can lead to:
-
Infection (bacterial contamination, abscesses, cellulitis)
-
Granulomas (inflammatory lumps requiring medical treatment)
-
Allergic reactions (from contamination or degraded product)
-
Product breakdown (filler loses efficacy and safety once exposed)
-
Serious complications (tissue necrosis in rare cases)
No discount is worth your health.
What Allergan & Merz Say
Allergan and Merz, the pharmaceutical companies that manufacture the dermal fillers we use, explicitly prohibit storing and reusing opened syringes.
From their safety guidelines:
-
Syringes are single-use only
-
Once opened, any unused product must be discarded immediately
-
Storing opened syringes for later use voids all product warranties and safety guarantees
We follow these guidelines without exception.
Why Some Practices Offer Split Syringes Anyway
We understand, filler is expensive, and splitting syringes can make it seem more affordable. So why do some practices do it?
Because they prioritize profit over safety.
Offering split syringes allows practices to:
-
Charge two patients for one syringe (doubling revenue per unit)
-
Attract price-sensitive customers with "discounted" half-syringe pricing
-
Compete on cost rather than clinical standards
We refuse to compromise patient safety to lower prices or increase profit margins.
Our Policy: Safety Over Savings
At PM Aesthetics:
-
Every syringe is used on one patient only
-
Any unused product is discarded immediately after your treatment
-
We never store opened syringes, even if you request it
-
We follow manufacturer safety protocols without exception
If you want half a syringe administered, we're happy to do that, but the remaining product will be discarded, not saved. This is not negotiable, because your safety is not negotiable.
Affordable Alternatives That Don't Compromise Safety
We understand that filler pricing can feel out of reach, especially if you're just starting your aesthetic journey. Here are safer ways to make lip enhancement more affordable:
-
Start with just a lip flip ($69)—gives you a subtle lift without the cost of filler
-
Save up and return when you're ready—we'd rather you wait and do it safely than rush into something risky
-
Join our membership program—members receive significant discounts on filler and all other services
-
Choose the PM Pout Package—bundled pricing saves you $159 and includes healing modalities
Questions to Ask If You're Shopping Around
If another practice offers "split syringes" or "half-syringe pricing," ask them:
-
"Do you split syringes between multiple patients?"
-
"How do you store opened syringes?"
-
"Do you follow Allergan/Merz manufacturer guidelines on single-use syringes?"
-
"Are your providers medically licensed and trained?"
If they hesitate, give vague answers, or try to reassure you that "refrigeration keeps it safe," that's a red flag.
No reputable, safety-focused practice will reuse opened syringes. Period.
Why Choose PM Aesthetics for Lip Filler?
1. Medical Expertise
All injections are performed by medically licensed providers (NPs, PAs, MDs) with advanced training in facial anatomy and dermal filler techniques.
2. Premium Products Only
We exclusively use FDA-approved, pharmaceutical-grade fillers from Allergan (Juvéderm) and Merz Aesthetics (Belotero), never compounded, off-brand, or discount products.
3. Natural, Balanced Results
We enhance your features, we don't erase them. Our providers specialize in creating lips that look naturally fuller and beautifully shaped, never overdone or disproportionate.
4. Provider Authority to Refuse
Our providers can, and will, refuse treatment if it's not in your best interest. If you're requesting too much filler, an unnatural result, or treatment too frequently, we'll have an honest conversation instead of handing you a consent form.
5. Transparent Pricing
No hidden fees, no surprise charges. The price we quote is the price you pay.
6. No Commissions
Our providers are salaried medical professionals whose compensation is never tied to how many syringes they use. Their recommendations are based solely on clinical appropriateness and your goals, not revenue targets.
FREQUENTLY ASKED QUESTIONS
Most patients achieve beautiful, natural results with 1 syringe (1 mL) of filler. This is enough to add noticeable volume, enhance shape, and create balanced proportions without looking overdone.
Some patients may want more volume and choose 1.5-2 syringes over multiple sessions, but we recommend starting conservatively. You can always add more, you can't take it away without dissolving it.
Natural results depend on two things: your provider's skill and your anatomy.
At PM Aesthetics, we specialize in creating lips that look like enhanced versions of your own, not like someone else's lips copied onto your face. We assess your facial proportions, natural lip shape, and aesthetic goals to create results that harmonize with your features.
If you request filler that would create an unnatural or disproportionate result, our providers will tell you, and may refuse treatment. We're not here to make you look like a screenshot from Instagram. We're here to make you look like the best version of you.
Most patients describe it as uncomfortable but tolerable.
We use:
-
Topical numbing cream before injection
-
Filler that contains lidocaine (numbs as we inject)
-
Ice or vibration tools to minimize discomfort
You'll feel pressure, pinching, and a slight burning sensation, but it's over quickly (5-10 minutes of actual injection time).
-
12-18 months on average, depending on:
-
The product used (Juvéderm lasts longer than Restylane in lips which is why we use Juvéderm)
-
Your metabolism (some people break down filler faster)
-
How much you move your lips (talking, kissing, eating all cause filler to metabolize faster)
-
The amount injected (more filler = longer-lasting results)
Lip filler metabolizes faster than filler in other areas (like cheeks) because lips are highly mobile.
-
Lip Flip
Neurotoxin (Botox/Xeomin)
Relaxes muscle above upper lip, causing it to "flip" outward
No added volume
Subtle enhancement
Lasts 2-3 months
$69
Best for: subtle lift, gummy smile
Lip Filler
Hyaluronic acid filler (Juvéderm/Restylane)
Adds volume directly into lips
Adds volume
Noticeable enhancement
Lasts 12-18 months
$699
Best for: volume, shape, definition
Many patients do both, lip flip enhances the shape while filler adds volume.
Yes! Popular combinations include:
Lip filler + lip flip ($69 enhancement)—enhances both shape and volume
Lip filler + neurotoxin (forehead, crow's feet, etc.)—refresh your entire face
Lip filler + skincare treatments (peels, facials)—maximize your results
We can perform multiple treatments in one visit to save you time.
Hyaluronic acid fillers (like Juvéderm and Belotero) are reversible.
If you're unhappy with your results, we can inject hyaluronidase, an enzyme that dissolves the filler within 24-48 hours. This is rare, but it's an option if:
-
Results are asymmetric or unnatural
-
You experience complications (vascular occlusion, severe swelling)
-
You simply change your mind
Hyaluronidase: $400 per vial
-
Yes, after the initial swelling subsides (7-14 days).
Immediately after treatment, your lips will feel:
-
Swollen and tight
-
Slightly firm or lumpy
-
Tender to touch
Within 2 weeks, filler integrates into your tissue and feels soft, natural, and like your own lips—just fuller.
-
Yes, but you'll need antiviral medication.
Lip injections can trigger cold sore outbreaks in people with a history of HSV-1 (oral herpes). We'll prescribe Valtrex (valacyclovir) to take before and after treatment to prevent an outbreak.
Let your provider know if you've ever had a cold sore—even years ago.
Wait 24 hours before applying lipstick, lip gloss, or any lip products. This allows the injection sites to heal and reduces infection risk.
After 24 hours, you can resume normal makeup application.
Bruising and swelling are normal and expected.
To minimize:
-
Avoid blood thinners (ibuprofen, aspirin, alcohol, fish oil) 48 hours before and after treatment
-
Use Arnica (topical cream or oral tablets) starting 2 days before treatment
-
Apply ice or cold compress for the first 24 hours
-
Sleep elevated (propped on pillows) the first night
Swelling peaks at 24-48 hours and resolves within 7-14 days.
Bruising typically fades within 7-10 days.If you have an event (wedding, photoshoot, etc.), book lip filler at least 2 weeks in advance.
-
Yes. PM Aesthetics members receive:
-
Discounted filler pricing (up to 35% off)
-
Preferred booking (priority scheduling)
-
Member-only promotions (exclusive offers throughout the year)
-
No credit card processing fees on in-clinic payments
